Keynote: Serum IRAP, a novel direct biomarker of prediabetes

Biography:

Serge Bottari, obtained his M.D. and his Ph.D. degrees at the Free University Brussels, Belgium. He specialized in OB/GYN and also obtained a PhD in Biochemistry. He was a Post-Doctoral Fellow and Research Associate at UC San Francisco from 1984 till 1988. After having been a project leader at Sandoz and CIBA-Geigy in Basle, Switzerland, he became Professor of Cell Biology at the Medical School and Head of Endocrine Biology at the University Hospital in Grenoble, France, in 1993. His publications in premium journals have been cited more than 4000 times and he is a member of several editorial boards. He also holds several patents. His current work focuses among others on the molecular mechanisms involved in insulin-resistance and on the development of novel diagnostic tools.

Abstract:

Insulin Resistance (IR), currently called prediabetes, affects more than half of the adult population worldwide. Type 2 diabetes (T2D), which often follows in the absence of treatment, affects more than 400 million people and represents more than 10 % of the health budget in industrialized countries. A preventive public health policy is urgently needed in order to stop this constantly progressing epidemic. Indeed, early management of prediabetes does not only strongly reduce its evolution towards T2D but also strongly reduces the appearance of cardiovascular comorbidity as well as that of associated cancers. There is however currently no simple and reliable test available for the diagnosis or screening of prediabetes and it is generally estimated that 20 to 60 % of diabetics are not diagnosed. We therefore developed an ELISA for the quantitative determination of a novel circulating biomarker of IR, IRAP (Insulin-Regulated Amino Peptidase, EC 3.4.11.3). IRAP is associated with and translocated in a stoechiometric fashion to the plasma membrane together with GLUT4 in response to insulin in skeletal muscle and adipose tissue. Its extracellular domain (IRAPs) is subsequently cleaved and secreted in the blood stream. In T2D, IRAP translocation in response to insulin is strongly decreased. Our patented sandwich ELISA is highly sensitive (≥ 10.000- fold "normal" fasting concentrations) and specific, robust and very cost-effective. Dispersion of fasting plasma concentration values in a healthy population is very low (101,4 ± 15,9 μg/ml) as compared to insulin and C-peptide. Results of pilot studies indicate an excellent correlation between IRAPs levels and insulin sensitivity. We therefore think that plasma IRAPs is a direct marker of insulin sensitivity and that the quantitative determination of its plasma levels should allow large-scale screening of populations at risk for predsiabetes and T2D, thereby allow the enforcement of a preventive health policy aiming at efficiently reducing this epidemic.

Biography:

Leelavinothan Pari has completed his PhD Biochemistry from University of Madras, Chennai. He is a Professor of Biochemistry, Faculty of Science, Annamalai University, Tamil Nadu, India. He has published more than 175 papers in reputed international journals and has been serving as an Editorial Board Member of repute. He is currently working on health promoting phytochemicals and diabetes. He has produced 25 PhD and 60 MPhil. He received many prestigious awards such as DBT-CREST Award (Visiting Professor, Cardiovascular Research Center, University of Glasgow, UK-2013), Commonwealth Academic Professional Fellowship (Visiting Scientist, Institute of Molecular Sciences, University of London, UK-2012), INSA Award (Visiting Scientist, German Diabetes Research Center, Dusseldorf, Germany-2007), Indian Academy of Science Summer Fellowship (Visiting  researcher, Central Drug Research Institute, Lucknow, India-2006), Indo- Polish Collaborative Research Award (Visiting Professor, Polish Academy of Science, Poland-2005) and DBT Overseas Associateship Award (Visiting Researcher, Division of Biomedical Sciences, Streathclyde University, Glasgow, UK-2004).

Abstract:

Insulin signaling pathway is an important role in glucose utilization in tissues. Our previous study has established that myrtenal has antihyperglycemic effect against diabetic rats. The aim of this study was to explore the molecular mechanism of myrtenal in Streptozotocin-induced diabetic rats. Experimental diabetes was induced by single intraperitoneal injection of Streptozotocin (STZ) (40 mg/kg bw) in Wistar albino rats. Diabetic rats were administered myrtenal (80 mg/kg bw) for a period of 28 days. Diabetic rats showed an increased the levels of plasma glucose, decreased the levels of plasma insulin, downregulation of insulin receptor substrate 2 (IRS2), Akt and glucose transporter 2 (GLUT2) in liver and insulin receptor substrate 2 (IRS2), Akt and glucose transporter 4 (GLUT4) protein expression in skeletal muscle. However, myrtenal treated diabetic rats revealed decreased the levels of plasma glucose, improved the plasma insulin levels, up-regulation of IRS2, Akt and GLUT2 in liver and IRS2, Akt and GLUT4 protein expression in skeletal muscle. The up-regulation of glucose transporters enhances
the glucose uptake in liver and skeletal muscle. The histopathology and immunohistochemical analysis of the pancreas also corroborates with the above findings. Our findings suggest that myrtenal could be a potent phytochemical in the management
of diabetes.

Abstract:

Adiponectin is an important adipokine with anti-inflammatory and insulin-sensitizing effects, the circulating levels of which are reduced in individuals with type 2 diabetes and obesity. Indeed, there is evidence that adiponectin released from Peri Vascular Adipose tissue (PVAT), that may play a key role in vascular physiology, as bioactive molecules released from it could have direct paracrine effects on the underlying vessel. The present study investigated the effects of extract Nigella sativa (N. sativa) on PVAT in diabetic mice.
Materials: Eight-week-old male musmusculus were treated with alloxan (150 mg/ kg) by a single intraperitoneal injection to induce diabetes mellitus. At 3 days of injection, N. sativa extract (150 and 300 mg/kg) was administered via gavage for 2 weeks.
Results: The extract of N.Sativa administration significantly reduced blood glucose level (P < 0.05). However, N.Sativa did not alter metabolic parameters, such as body weight and food intake. The extract of N.Sativa administration sativa decreased the PVAT weight compared with diabetic group significantly (P<0,05). Furthermore,histology analyses demonstrated that N.Sativa decreased the inflammation cells in PVAT.
Conclusion: Extract of Nigella Sativa improved blood glucose level with attenuation of the inflammation in PVAT.

Gurudatta Amin is an Eminent Practitioner in the field of  multidisciplinary clinical cardiology. He has completed his Graduation in Ayurveda Medicines and has certificate in Six Sigma Blackbelt and PG scholar of Samhita Siddhant. He has partcicpated in many global conferences.

Abstract:

Around 20% of the population are in pre-hypertension stage and nearly 40% of the adult population from Hypertension (HTN). Blood Pressure Management Program (BPMP) is an Ayurvedic treatment strategy using Panchakarma. This study was conducted to evaluate the effect of BPMP on Systolic Blood Pressure (SBP), Diastolic Blood Pressure (DBP), Mean Arterial Pressure (MAP), Body Mass Index (BMI) and dependency on conventional therapy in elderly male patients of HTN. This observational study was conducted from January 2017 to February 2018, where in the data of HTN patients who attended Madhavbaug clinics in Maharashtra, India were identified. Data of patients who were administered BPMP (60-75 minutes) with minimum 6 sittings over 90±15 days were considered. Variables were compared between day 1 and day 90 of BPMP. Out of 29 enrolled patients, 24 were finally selected for analysis. BPMP showed significant improvement in SBP by 24.66 from 150.67
12.97 to 126±13.01 (p<0.001), DBP by 10.8 (from 87.7917±7.72 to 76.917±7.59 (p<0.001), MAP by 15.4 (from 108.75±7.14 to 93.25±8.72 (p<0.001). BMI (25.7275±2.63 kg/m2 to 24.91±2.32 kg/m2), also showed significant reduction. Dependency on concomitant medicines was reduced with the number of patients on no concomitant medicines increasing from 19 to 29%. BPMP can serve as an effective therapeutic regiment to combat HTN in elderly male patients.

Abstract:

Background: Lipid accumulation in the liver and pancreas is primarily caused by combined hyperlipidemia. Syzygium polyanthum (Wight) Walp. var. polyanthum leaves are consumed as a traditional Indonesia treatment. However, the effect of extract Syzygium polyanthum (Wight) Walp in isolated hypercholesterolemia is not fully described. Therefore, our aim was
to investigate whether extract Syzygium polyanthum (Wight) Walp decreased the hypercholesterolemia leads to alterations in histology of hepatic lipid in Wistar rats.
Methods and Results: Hypercholesterolemia in Wistar rat was induced by oral administrated of high fat diet (HFD) of lard (2ml/200g). Two weeks after HFD, the extract Syzygium polyanthum (Wight) on dose dependent (0.72mg/kg/day, 0.9 mg/ kg/day, and 1.08 mg/kg/day) was administrated via oral gavage for 4 weeks. Treatment with extract of Syzygium polyanthum (Wight) reduced cholesterol total levels (P<0.05) in hyperglycemic Rat model. Also, decreased the others metabolic parameters high-density lipoprotein (HDL) (P<0.05), and triglyceride (TG) (P<0.05) with alteration of body weight and blood glucose. Furthermore, the histology analysis demonstrated that, in hypercholesterolemia rat model, the extract of Syzygium polyanthum (Wight) reduced the isolated hypercholesterolemia lipid accumulation in liver (p < 0.05).
Conclusion: Cholesterol-total lowering effects of extract Syzygium polyanthum (Wight) were associated with the reduction of lipid accumulation in the liver in hypercholesterolemia Wistar Rat model. These results may provide a possible mechanism for hepatoprotective and cardioprotective effects of extract of Syzygium polyanthum (Wight).

Abstract:

Globally, Diabetes Mellitus (DM) prevalence has created menace, being major culprit of increased mortality and morbidity and health care expenditures. India is 2nd country with maximum number of diabetic patients, with an estimated prevalence of around 10%. Comprehensive Diabetes Care (CDC) is a combination of Panchakarma and diet management. This study was conducted to evaluate the effect of CDC on glycosylated haemoglobin (HbA1c), Body Mass Index (BMI), body weight, abdominal girth and dependency on conventional therapy in DM Patients.This retrospective study was conducted in from July 2017 to January 2018, where in the data of elderly male type 2 DM patients (HbA1c>6.5%) who attended Madhavbaug clinics in Maharashtra, India were identified. Data of patients who were administered CDC (60-75 minutes) with minimum 6 sittings over 90±15 days were considered. Variables were compared between day 1 and day 90 of CDC. Out of 48 enrolled elderly male patients, 34 were included for analysis. CDC showed significant improvement in HbA1c from 8.27±0.96 to 7.1±1.30; (p=0.0001), BMI from 27.65±3.20 to 25.91±3.29; (p<0.0001), weight from 73.75±10.76 to 69.46±10.39; (p<0.0001). Abdominal girth (from 100.0±9.08 to 95.36±9.10; (p<0.0001), also showed significant reduction. Dependency on concomitant medicines was reduced, with number of patients on no concomitant medicines increasing from 3-15%, CDC and allopathy both are found to be efficacious; but CDC acts dually, by reducing HbA1c as well as reducing dependency on allopathic medications.

Abstract:

For 60 years since its founding, Kerala took pride as the number one State in the country on life expectancy, receiving praise from world renowned global health expert, professor Hans Rosling. Professor Rosling puts Kerala ahead of Washington D C on Human Health Index at a fraction of per capita income. However, lifestyle diseases like diabetes is now in the order of an epidemic in the state reducing the life expectancy of our adult population above the age group of 30. For the first time Kerala lost out its number one position on life expectancy to Jammu and Kashmir due to an alarming drop in life expectancy after the age of 30 compared to J&K as per 2010-14 census data. Root cause of our problem with diabetes and lifestyle diseases is the rapid increase of starch in our diet over the past 60 years through rice, wheat, and tubers like cassava. Through years
of policy interventions we were able to make starch more affordable but the over correction is now hurting our health than helping it. In order for Kerala to get back and retain our number one position in life expectancy, we have to cut back our starch consumption from half a plate to a quarter and increase consumption of our grossly underutilized green jackfruits. The presentation scientifically explains why one of Kerala’s oldest traditions of eating green jackfruit as a meal is the perfect plate for diabetes and lifestyle diseases. The result states from the first glycemic research conducted on green jackfruit at Sydney University, first randomized control trial on green jackfruit meal showing reduction in HbA1c and finally shows new empirical evidence on a dramatic drop in diabetes medicine sales in Kerala during the first jackfruit season after it was elevated as the State’s official fruit.
